Our clinical support services help older people manage health needs safely and confidently at home. This includes nursing care, continence support and access to allied health professionals, delivered in line with individual care plans and assessed needs. Care is delivered by qualified professionals, including enrolled nurses, with a strong focus on safe medication support, recovery after hospital stays, rehabilitation and palliative care. When required, we provide round-the-clock care at home and specialised support for people living with memory loss, including dementia-related conditions. Clinical support services are fully funded by the Australian Government under the Support at Home program, with no out-of-pocket contribution for eligible participants.
